Pick up application at 302 W Main St. (corner of 5th and Main) on November 15 or 17 or 22 between 9:00am and 12:00pm
Email the information below any time before November 25 to greg@stcabrini.org
ALL INFORMATION MUST BE COMPLETE AND SIGNED OR YOUR NAME WILL GO TO THE BACK OF THE LINE. APPLICATIONS MUST BE TURNED IN NO LATER THAN DECEMBER 1.
YOU WILL NEED:
1. Photo identification for ALL adults in the home 18 and over
2. Verification of household home address
3. Verification of dates of birth for children 16 and under
4. Your current bill for electricity, water, natural gas or phone (receipt with address and ID)
